
STEP 1
MAKE AN APPOINTMENT WITH THE AUSTIN FIRE DEPARTMENT'S ADMINISTRATIVE
OFFICES ("ADMIN") AT (512) 974 - 4100.
They will provide you with the appropriate paperwork that needs to be completed including:
- Resignation Form (For purposes of retirement)
- Exit Interview Form
Your termination date or last active day can be determined after meeting with "admin" and that information will be forwarded to the pension office.
STEP 2
MAKE AN APPOINTMENT WITH THE PENSION OFFICE AT (512) 454 - 9567. YOU
MAY CALL BILL STEFKA OR LINDA ADNEY AT THIS NUMBER.
The following information will be necessary when applying for retirement:
- Notification from Fire Department
- Copy of Drivers License
- Copy of Marriage License (If Applicable)
- Drop Application and Election
- Distribution Election Form
- Optional Retirement Annuity Election
- Withholding Notice
- Direct Deposit Form
- City of Austin Medical Insurance
- City of Austin Dental Insurance
- HELPS Enrollment Form
- Home Phone Number
- Home Address
- Month of 1st Annuity Check
